Read these Terms and Conditions (this “Agreement”) for important information about our text alert services (“Pharmalife Community Pharmacy, LLC text alerts”). THIS AGREEMENT CONTAINS A MANDATORY ARBITRATION OF DISPUTES PROVISION THAT REQUIRES THE USE OF ARBITRATION ON AN INDIVIDUAL BASIS TO RESOLVE DISPUTES, RATHER THAN JURY TRIALS OR CLASS ACTIONS.
**Text Message Terms and Conditions**
By subscribing to our text messaging service, you agree to the following terms and conditions:
1. **Service Description**: By providing your phone number, you consent to receive automated and manual text messages from Pharmalife Community Pharmacy regarding your medications, appointment reminders, health tips, promotions, and other related information.
2. **Frequency**: You may receive multiple messages per week, depending on your interactions with us.
3. **Message and Data Rates**: Standard message and data rates may apply. You are responsible for any charges incurred from your mobile carrier.
4. **Opt-Out**: To stop receiving text messages, reply with “STOP” to any message. You will receive a confirmation message indicating that you have been unsubscribed. Please note that opting out may affect your ability to receive important prescription notifications.
5. **Help**: For assistance or more information, reply with “HELP” or contact us at Pharmalife Community Pharmacy. We are here to help you with any questions or concerns.

7. **Liability**: Pharmalife Community Pharmacy is not responsible for any delays or failures in message delivery. We are also not liable for any errors in the information provided via text messages. Messages are not encrypted and secure and may contain personal health information. Those with access to your phone may have contact to all information on your phone, including Pharmacy Information. We are not liable if someone obtains your phone and obtains your information.
8. **Changes**: We reserve the right to modify these terms and conditions at any time. Any changes will be communicated to you via text message.
By opting into our text messaging service, you acknowledge that you have read, understood, and agreed to these terms and conditions.

With the exception of any of the language above in this Dispute Resolution provision relating to the waiver of class and representative actions, if a court decides that any part of this Dispute Resolution provision is invalid or unenforceable, the other parts of this Dispute Resolution provision shall still apply. If a court decides that any aspect of the language above in this Dispute Resolution provision relating to the waiver of class and representative actions is invalid or unenforceable, then the entirety of this Dispute Resolution provision shall be null and void. The remainder of the Agreement will continue to apply and be unaffected by this severability provision.
